This case report describes the non-surgical management of a large cyst-likeperiapical lesion in the maxillary anterior region of a 21-year-old female with the chief complaintof persistent, chronic and dull aching pain and pus drainage through sinus opening in relation to the maxillary left central incisor. Root canal treatment of both left maxillary central and lateral incisors were initiated. Calcium hydroxide dressing was placed monthly for a period of 3 months. After 3months, obturation was carried out on both teeth. Clinical and radiographic re-evaluations at 6 and 9months revealed progressing bone healing. This case report shows that appropriatediagnosis in combination with root canal treatment as a conservative non-surgicalapproach can lead to complete healing of large lesions without invasive surgical treatment.
