Sarcomatoid carcinomas are rare tumors of oral cavity. Thrse tumors are found commonly at other sites of the body but in the oral cavity, the presence of sarcomatoid carcinoma is very rare. Only few cases had been reported in the literature. Here we report a rare case of sarcomatoid carcinoma arising from the lining of the orthokeratenized odontogenic cyst. Sarcomatoid carcinoma is a biphasic tumor. It is made up of cells from squamous and spindle cells carcinoma with a sarcomatous appearance, which is of epithelial origin. The diagnosis of sarcomatoid carcinoma often presents a clinical-pathological challenge where immunohistochemical technique (IHC) plays a key role in the histopathological diagnosis. In this work we present one case of sarcomatoid carcinoma where the use of IHC allowed us to achieve a conclusive diagnosis.
