Evaluation of the therapeutic outcome of Intra Articular Injection of Sodium Hyaluronate following Orthocentesis in internal derangement of Temporomandibular Joint

Objective: The study aims to evaluate the therapeutic outcome of intra articular injection of sodium hyaluronate following orthocentesis in internal derangement of temporomandibular joint. Study Design: Twenty patients with TMJ internal derangement underwent arthrocentesis using ringer lactate followed by intrarticular injection ofsodium hyalouronate.Pain using visual analog scale, maximum mouth opening were documented pre-operatively and post-operatively. Patients were followed for 6 months. Statistical analysis of pain and maximum mouth opening was done by calculating P value (ANOVA). Results: The mean pre-operative pain was 6.6 ± 1.52 and post-operatively at 6th month was 0.5 ± 1.14with an average decrease of 6.0 (P = 0.000). The mean maximal mouth opening pre-operatively was 26.14 ± 4.12 mm and post-operatively 38.9 ± 3.23 mm at 6 month. The mean increase in the mouth opening was 12.7 mm (P = 0.000). Conclusion: Arthrocentesis is an intermediate procedure between non- surgical and surgical treatment of TMJ internal derangement. Tmj internal derangement is often related to the failure of lubrication system of joint, thus orthrocentesis supplemented by intra articular injection of sodium hyaluronate procedure has  significant benefits in patients with TMJ internal derangement.
